A wealthy industrialist hires Black Jack to perform a complicated operation on his son. The doctor reluctantly agrees, but his heart breaks when he discovers the younger brother was sent away to study medicine in order to fulfil their father’s wishes.
After a car accident, a man whose arm was lost seeks out Black Jack to replace it. The surgeon agrees, but he warns him that he may not be as good as the original owner. The man, a businessman, is shocked when the limb feels just as natural as his own arm.
The reckless son of a billionaire hires Black Jack to save his life. The surgeon is able to save the boy’s leg, but the child dies shortly afterwards. The father, Nikla, hires Black Jack to transplant his ill son’s organs into a donor. Black Jack tells him that he can only save his son using body parts from an unknown donor. The wealthy Nikla hires the reckless delinquent Davy as his donor.
As he and Pinoko are traveling by train, Black Jack receives an urgent message from his former mentor who is in Africa. Despite the warnings of others, Black Jack ignores them and travels to Africa. Upon arrival, he finds the man in a mysterious state. The plague had affected his mental capacity, but Black Jack successfully creates a cure.
A young woman named Sono has a weak heart and is sensitive to light. Her family’s main focus is ikebana so she is pushed to the brink of death to become better. When she almost dies while trying to see wildflowers, Black Jack tells her that flowers and humans are both beautiful because they live.
When a police officer has his statue vandalised by thugs with a vendetta against him, he stays hidden in the night to capture them red handed. Black Jack, who has a past history with the officers, volunteers to help. The officer is grateful but also wants Black Jack to repair his reputation in the process. The two become friends and eventually fall in love. Despite their differences, they are able to overcome the obstacles and become a couple.
